I. SKANY WYMAGANE NA ETAPIE REJESTRACJI

1) CV z opisem zamierzeń badawczych i wykazem dotychczasowego dorobku naukowego kandydata (wzór w systemie IRK);

2) opinia potencjalnego promotora¹, dotycząca zamierzeń badawczych i dotychczasowego dorobku naukowego kandydata, wraz z opinią dziekana właściwego wydziału Uniwersytetu Gdańskiego dotyczącą możliwości realizacji w danej jednostce Uniwersytetu Gdańskiego projektu badawczego proponowanego przez kandydata (wzór w systemie IRK);

3) własnoręcznie podpisane oświadczenie kandydata, że nie jest doktorantem w innej szkole doktorskiej, wygenerowane w systemie IRK (wygenerowane w systemie IRK);

4) dyplom ukończenia studiów w zagranicznej uczelni, uprawniający do ubiegania się o nadanie stopnia naukowego doktora w państwie, w którego systemie szkolnictwa wyższego działa uczelnia, lub inny dokument ukończenia studiów za granicą, uznany, zgodnie z przepisami w sprawie nostryfikacji dyplomów ukończenia studiów uzyskanych za granicą lub na podstawie umowy międzynarodowej, za równoważny polskiemu dyplomowi ukończenia studiów drugiego stopnia lub jednolitych studiów magisterskich wraz z tłumaczeniem na język polski sporządzonym i poświadczonym przez tłumacza przysięgłego;
dyplom lub inny dokument powinien być opatrzony poświadczeniem w formie:
a) apostille – jeżeli dokument został wydany przez instytucję działającą w systemie edukacji państwa będącego stroną Konwencji Haskiej z dnia 5 października 1961 r. znoszącej wymóg legalizacji zagranicznych dokumentów urzędowych (Dz. U. z 2005 r. Nr 112, poz. 938),
b) legalizacji – w pozostałych przypadkach;

5) wykaz ocen uzyskanych w toku studiów lub suplement do dyplomu ukończenia studiów drugiego stopnia lub jednolitych studiów magisterskich wraz z tłumaczeniem na język polski sporządzonym i poświadczonym przez tłumacza przysięgłego;

6) świadectwo szkolne, świadectwo dojrzałości lub dyplom ukończenia studiów, na podstawie którego kandydat został przyjęty na studia za granicą (ukończenie tych studiów jest potwierdzone wydaniem dyplomu, o którego uznanie za równoważny polskiemu dyplomowi ubiega się kandydat);

7) dokumenty potwierdzające dorobek naukowy (osiągnięcia naukowe), kwalifikacje lub inne osiągnięcia kandydata, w szczególności: kopia publikacji, certyfikaty językowe, zaświadczenia potwierdzające przyznanie nagrody naukowej, odbycie stażu lub członkostwo w kole naukowym; w przypadku dokumentu sporządzonego w języku innym niż polski lub angielski, kandydat składa również odpis tego dokumentu w tłumaczeniu na język polski lub angielski;

8) wypełniony i podpisany formularz osiągnięć kandydata (wygenerowany w systemie IRK). 

II. DOKUMENTY W POSTACI PAPIEROWEJ WYMAGANE PO OGŁOSZENIU LISTY KANDYDATÓW

Kandydat uprawniony do złożenia dokumentów dostarcza do właściwej jednostki zajmującej się rekrutacją do szkoły doktorskiej oryginały albo ich kopie poświadczone za zgodność z oryginałem przez: uczelnię, której kandydat jest absolwentem, notariusza lub członka komisji rekrutacyjnej.

Jednocześnie kandydat przedstawia do wglądu:

- dokument tożsamości,

- Kartę Polaka – w przypadku cudzoziemca będącego posiadaczem tego dokumentu.

1) własnoręcznie podpisane przez kandydata podanie o przyjęcie do szkoły doktorskiej (wygenerowane w systemie IRK);

2) CV z opisem zamierzeń badawczych i wykazem dotychczasowego dorobku naukowego kandydata (wzór w systemie IRK);

3) opinia potencjalnego promotora¹, dotycząca zamierzeń badawczych i dotychczasowego dorobku naukowego kandydata, wraz z opinią dziekana właściwego wydziału Uniwersytetu Gdańskiego dotyczącą możliwości realizacji w danej jednostce Uniwersytetu Gdańskiego projektu badawczego proponowanego przez kandydata (wzór w systemie IRK);

4) własnoręcznie podpisane oświadczenie kandydata, że nie jest doktorantem w innej szkole doktorskiej, wygenerowane w systemie IRK (wygenerowane w systemie IRK);

5) dyplom ukończenia studiów w zagranicznej uczelni, uprawniający do ubiegania się o nadanie stopnia naukowego doktora w państwie, w którego systemie szkolnictwa wyższego działa uczelnia, lub inny dokument ukończenia studiów za granicą, uznany, zgodnie z przepisami w sprawie nostryfikacji dyplomów ukończenia studiów uzyskanych za granicą lub na podstawie umowy międzynarodowej, za równoważny polskiemu dyplomowi ukończenia studiów drugiego stopnia lub jednolitych studiów magisterskich wraz z tłumaczeniem na język polski sporządzonym i poświadczonym przez tłumacza przysięgłego;

8) dokumenty potwierdzające dorobek naukowy (osiągnięcia naukowe), kwalifikacje lub inne osiągnięcia kandydata, w szczególności: kopia publikacji, certyfikaty językowe, zaświadczenia potwierdzające przyznanie nagrody naukowej, odbycie stażu lub członkostwo w kole naukowym; w przypadku dokumentu sporządzonego w języku innym niż polski lub angielski, kandydat składa również odpis tego dokumentu w tłumaczeniu na język polski lub angielski;

9) wypełniony i podpisany formularz osiągnięć kandydata (wygenerowany w systemie IRK);

10) uzupełniony oraz własnoręcznie podpisany przez kandydata wykaz dokumentów wymaganych w rekrutacji do szkoły doktorskiej , które zostały złożone przez kandydata (wygenerowany w systemie IRK).

III. DECYZJA „WARUNKOWA”

Kandydat do szkoły doktorskiej, który w trakcie postępowania wgrał do systemu komplet wymaganych dokumentów może zostać przyjęty do szkoły doktorskiej pod oznaczonym w decyzji warunkiem złożenia oryginałów dokumentów lub poświadczonych za zgodność z oryginałem kopii tych dokumentów – w ciągu 30 dni od ogłoszenia listy przyjętych, do siedziby komisji.

W przypadku niespełnienia powyższego warunku, Rektor Uniwersytetu Gdańskiego stwierdza wygaśnięcie decyzji, na podstawie której kandydat został przyjęty do szkoły doktorskiej.

¹ W przypadku ubiegania się o przyjęcie do szkoły doktorskiej w więcej niż jednej dyscyplinie naukowej kandydat składa opinie potencjalnych promotorów reprezentujących poszczególne dyscypliny.

I. SCANS REQUIRED AT THE REGISTRATION STAGE

1. a CV including a description of the candidate’s planned research and a list of the candidate’s academic achievements to date (template available in the IRK system);

2. an opinion issued by the prospective supervisor1 concerning the candidate’s planned research and academic achievements to date, together with an opinion of the Dean of the relevant faculty of the University of Gdańsk regarding the possibility of carrying out the research project proposed by the candidate within the relevant unit of the University of Gdańsk (template available in the IRK system);

3. a handwritten-signed declaration by the candidate stating that they are not a doctoral student at another doctoral school, generated in the IRK system;

4. a diploma of completion of studies at a foreign higher education institution, entitling the holder to apply for the award of a doctoral degree in the country whose higher education system includes that institution, or another document confirming completion of studies abroad, recognized — in accordance with the regulations on the nostrification of foreign higher education diplomas or under an international agreement — as equivalent to a Polish diploma of completion of a second-cycle programme or a long-cycle Master’s degree programme, together with a translation into Polish prepared and certified by a sworn translator;
   the diploma or other document must bear certification in the form of:
   a) an apostille — if the document was issued by an institution operating within the education system of a state that is a party to the Hague Convention of 5 October 1961 Abolishing the Requirement of Legalisation for Foreign Public Documents,
   b) legalization — in all other cases;

5. a transcript of records obtained during the course of study, or a diploma supplement to the diploma of completion of a second-cycle programme or a long-cycle Master’s degree programme, together with a translation into Polish prepared and certified by a sworn translator;

6. a school leaving certificate, secondary school certificate, or higher education diploma on the basis of which the candidate was admitted to studies abroad (the completion of those studies being confirmed by the issuance of the diploma for which the candidate is seeking recognition as equivalent to a Polish diploma);

7. documents confirming the candidate’s academic output (academic achievements), qualifications, or other accomplishments, in particular: a copy of publications, language certificates, certificates confirming the award of an academic prize, completion of an internship, or membership in a student research club; where a document has been issued in a language other than Polish or English, the candidate shall also submit a copy of that document translated into Polish or English;

8. a completed and signed candidate achievements form (generated in the IRK system).

II. PAPER DOCUMENTS REQUIRED AFTER THE ANNOUNCEMENT OF THE LIST OF CANDIDATES

A candidate entitled to submit documents shall provide to the relevant unit responsible for doctoral school admissions either the originals or copies certified as true copies of the originals by: the higher education institution from which the candidate graduated, a notary public, or a member of the admission committee.

At the same time, the candidate shall present for inspection:

• an identity document,

• a Polish Card (Karta Polaka) — in the case of a foreign national who holds this document.

1. an application for admission to the doctoral school, signed by hand by the candidate (generated in the IRK system);

2. a CV including a description of the candidate’s planned research and a list of the candidate’s academic achievements to date (template available in the IRK system);

3. an opinion issued by the prospective supervisor1 concerning the candidate’s planned research and academic achievements to date, together with an opinion of the Dean of the relevant faculty of the University of Gdańsk regarding the possibility of carrying out the research project proposed by the candidate within the relevant unit of the University of Gdańsk (template available in the IRK system);

III. “CONDITIONAL” DECISION

A candidate for admission to a doctoral school who has uploaded a complete set of the required documents to the system during the admission procedure may be admitted to the doctoral school subject to the condition specified in the decision that they submit the original documents or copies certified as true copies of the originals within 30 days of the announcement of the list of admitted candidates to the office of the admission committee.

If the above condition is not fulfilled, the Rector of the University of Gdańsk shall declare the expiry of the decision on the basis of which the candidate was admitted to the doctoral school.

¹ In the case of applying for admission to a doctoral school in more than one academic discipline, the candidate shall submit opinions from prospective supervisors representing the respective disciplines.
